Congratulations 2010 JDHS Graduates!

1..       Print your name exactly as you want it to appear on your name cards.  The recommended form is Michelle A. Ward using the middle initial for your middle name.

2.       Fill in your school name, home address, phone numbers, email address and other information.

3.       To order a package plan, check the box for the package you want and fill in the price in the right column.  Also, check the colors and styles for accessory items included in your package.

4.       To order individual items, or add to a package, mark the quantity and style for each item you would like and write in the price.

5.       At the bottom of the form, add all the items together in the SUBTOTAL. Then add the HANDLING and SALES TAX to the subtotal for the TOTAL.

6.       If a credit card (VI,MC,Diners, Discover, Amex) is being used for payment, use space to write the complete card number, expiration date and name shown on the card. Credit card charges must be signed by the cardholder in the same area.
